Membership Commitment Form

I understand that I am making a commitment to 100MKL to make an annual donation of $408 ($102 x 4 meetings or prepaid annually). If writing a cheque, I will write it to Community Foundation of Kawartha Lakes. I agree to fulfill my donation commitment even if I did not vote for the charity selected by majority vote.

If I am unable to attend a quarterly meeting before the meeting, I will give my payment to another member to deliver in my place along with my proxy vote.

• For e-transfers, please send donations to info@kawarthafoundation.ca , then send a separate email with your name, address, email, donation amount and directing to the 100 Men Who Care KL Fund.
• For all cheque donations, please indicate "100 Men Who Care KL" in the memo line of your cheque. Tax receipts will be made out to the name & address indicated on the cheque, using the email address on file
• Or mail it to: 100 Men of Kawartha Lakes, c/o Community Foundation of Kawartha Lakes, 165 Kent St W, Suite 302, Lindsay, Ontario K9V 2Y5. (Please indicate "100 Men Who Care KL" in the memo line of your cheque)

• When using a credit card, I agree to add 3% to cover credit card fees

By signing this form, I release 100 Men of Kawartha Lakes and all its members, volunteers, and participants from any liability for injury, loss, or damage to personal property associated with activities or events organized by the group. This release includes all claims, whether arising from negligence or otherwise, to the fullest extent permitted by Ontario law. I understand that this waiver does not affect my rights under the Ontario Consumer Protection Act, 2002, where applicable. I acknowledge that my name and picture may appear on the 100MenKawarthaLakes.ca website, or social media platforms. Information will only be used for 100 Men Kawartha Lakes.
